Your 1st Appointment
The staff of Charleston Nephrology are committed to making your time with us efficient and enjoyable. See some helpful tips and download forms that you can complete prior to your arrival below.
- Please bring all medications including over the counter supplements or a completed list to EACH appointment.
- If you have had labs done with another physician within 30 days of your appointment, please request those labs be faxed to our office or bring a copy with you. Labs may be faxed to 843-767-9313.
- Please expect to give a urine sample on the day of your appointment.
- Please allow 48 hours to process RX refill requests.
- New Patient Form (complete prior to arrival)
